The photo and concept is interesting although it took me a bit to connect the dental tool, the death character and plaque. The reason is at first glance, I seemed to have connected the death character, which is the first thing I notice, with the dental tool. This subconsiously put the dental tool as a negative. Then when I read the tag line below, which is not that prominent, I started to connect the dental tool as a negative for plaque.

The problem here is that I did initially connect the dental tool with the death character as a negative in general which is not the intention of the advertiser. So in this case, an ad where let's say some type of superhero or other positive character holding the dental tool, probably would have worked better for me in terms of connecting all the elements in the intented way faster.
